whoopps, I completely misunderstood what you mean sorry. And yes this is one of the persistent issues within in-org feedback, both outside AD&T and within AD&T (feedback originating from QA for example)

I think it could be even done over chair, or committee email as long as it is documented as a formal request and passed through some form of internal process. However I like your idea (it is similar to that of systems process with answering tickets)

I also do think this needs to be in addition to archive management checking all venues of feedback, internal and external more thoroughly. Both through case studies, actively asking for feedback (both on specific projects and on noticed points of improvement overall to put into the schedule).
